To apply for Birth Certificate you will need: your mailing address, phone number and your first and last name or single name; information about the person named on the certificate, which includes parent’s information, all given names, last or single name, sex, date and city of birth; a guarantor, if the subject is 9 years or older; and the payment for the certificate.
